This form is so we, First Baptist Church staff, have all parental information for every child in our care along with medical information. In the case of an emergency, we are aware of any prior conditions and can help the child to the best of our abilities. The media release is so First Baptist Church has permission to take photos and/or videos of your child at an event. It also gives First Baptist Church staff permission to post pictures and/or videos on our website and on our social media pages. Both are to ensure the safety of your children, as that is our first priority in the NextGen ministry and First Baptist Church.
